Connected Care, Wherever You Are
“Our family includes adult kids and relatives living in three different time zones and one studying abroad. How will we manage if someone goes to the hospital for emergency care?”
This question expresses every modern family’s need for a trusted person to take charge of coordinating long distance communications and decision-making when a loved one’s health takes an unexpected turn.
As your patient advocate, I can act as primary point of contact, arranging virtual family team meetings, coordinating communications with doctors, hospitals, and family members, and arranging local, in-person support and services. I can also send updates and summaries via email or group chat to geographically distant family members and provide supportive guidance to family members who are present.
How Spark can help:
Coordinate communications between the patient, their doctors and family members
Act as neutral third party to gather and share information to help patients and loved ones make decisions about their care
Ensure key documents are in place (Health Care Proxy, etc.) and easily accessible
“I’ve watched Pamela navigate care and guide health decisions for family, friends and her parents in their later years. She is compassionate, caring and kind – someone I implicitly trust to have my best interests in mind.”
